Trach/Vent Nurse (LPN)

Qualifications:

  • Must have a valid LPN license in the respective state.
  • Minimum of 6 months of hands-on nursing experience.
  • Current CPR certification.
  • Willingness to provide in-home nursing care to infants, children, and adolescents.

Responsibilities:

  • Provide one-on-one patient care in a home environment.
  • Execute physicians' care plans and document care provided accurately.
  • Administer prescribed medication, treatments, and therapies.
  • Conduct patient assessments, coordinate care, and educate family members.

Position Overview: The Trach/Vent Nurse (LPN/LVN) provides skilled nursing care following care plans and physician orders, under RN supervision.

Essential Job Functions:

  • Quality patient care delivery in alignment with physician orders.
  • Continuous patient assessment and reporting changes in condition.
  • Timely documentation of care activities and nursing actions.
  • Implementation and updating of nursing care plans.
  • Infection control, patient education, and continuous professional development.

Requirements:

  • Graduate of an accredited nursing school.
  • Current LPN license in the state of practice.
  • CPR certification and proficiency in clinical assessments and documentation.
  • State-specific experience as per requirements.

Preferences:

  • Minimum of 6 months recent nursing experience.
  • Home health care experience.

Other Skills/Abilities:

  • Detail-oriented, time management, problem-solving.
  • Strong organizational and communication skills.

Physical Requirements:

  • English proficiency, ability to travel, lift 50 pounds, and reposition patients.
  • Standing, bending, kneeling, and adequate sensory abilities.
  • Response to emergencies, exposure to varied environments.

Environment:

  • Function in diverse environments with potential exposure to allergens.
  • Possible exposure to bodily fluids and infectious diseases.
