Trach/Vent Nurse (LPN)

Qualifications:
- Must have a valid LPN license in the respective state.
- Minimum of 6 months of hands-on nursing experience.
- Current CPR certification.
- Willingness to provide in-home nursing care to infants, children, and adolescents.
Responsibilities:
- Provide one-on-one patient care in a home environment.
- Execute physicians' care plans and document care provided accurately.
- Administer prescribed medication, treatments, and therapies.
- Conduct patient assessments, coordinate care, and educate family members.

Position Overview: The Trach/Vent Nurse (LPN/LVN) provides skilled nursing care following care plans and physician orders, under RN supervision.
Essential Job Functions:
- Quality patient care delivery in alignment with physician orders.
- Continuous patient assessment and reporting changes in condition.
- Timely documentation of care activities and nursing actions.
- Implementation and updating of nursing care plans.
- Infection control, patient education, and continuous professional development.
Requirements:
- Graduate of an accredited nursing school.
- Current LPN license in the state of practice.
- CPR certification and proficiency in clinical assessments and documentation.
- State-specific experience as per requirements.
Preferences:
- Minimum of 6 months recent nursing experience.
- Home health care experience.
Other Skills/Abilities:
- Detail-oriented, time management, problem-solving.
- Strong organizational and communication skills.
Physical Requirements:
- English proficiency, ability to travel, lift 50 pounds, and reposition patients.
- Standing, bending, kneeling, and adequate sensory abilities.
- Response to emergencies, exposure to varied environments.
