Job Description
Join our team at Heartland Hospice, a member of the Gentiva family, as a full-time RN Case Manager. In this role, you will provide compassionate and personalized care to hospice patients and their families. You will act as a primary liaison between physicians, patients, and the hospice team, ensuring comfort, peace, and dignity.
Responsibilities
- Provide direct patient care services and supportive care to patients and families.
- Empower caregivers through education on patient care.
- Collaborate with interdisciplinary teams to ensure comprehensive care.
- Evaluate patient needs and execute personalized care plans.
- Maintain accurate patient clinical records and participate in quality assessment programs.
Requirements
- RN license in the state of employment.
- 1+ years of nursing experience, preferably in hospice or related fields.
- Compassionate, empathetic, and adaptable to crisis situations.
- Current driver's license, automobile insurance, and CPR certification.
