RN Case Manager - Full-Time - Hospice ($5000 Sign On)

Job Description

We are looking for a compassionate and experienced RN Case Manager to join our team at Heartland Hospice. As a Full-Time Hospice RN, you will play a crucial role in providing direct patient care services and support to patients and their families during challenging times. Responsibilities

• Report directly to the Administrator, Executive Director, or Patient Care Manager Senior.
• Act as the primary liaison between physicians, patients/families, and the hospice team.
• Provide direct patient care services and supportive care.
• Empower patient caregivers through education and support.
• Collaborate with an interdisciplinary group to ensure comfort, peace, and dignity.
• Assist in developing and executing patient care plans.
• Maintain accurate patient clinical records.
• Participate in quality assessment and improvement programs.

Requirements

• Compassionate and empathetic towards patients and families.
• RN license in the state of employment (or eligibility to obtain).
• 1+ years of nursing experience preferred.
• Additional experience in areas like hospice, home health, ICU, or oncology is a plus.
• Current driver's license and automobile insurance.
• Current CPR certification.
