Job Description
Heartland Hospice, a member of the Gentiva family, is seeking a compassionate and empathetic RN Case Manager to join our team on a full-time basis. In this role, you will provide direct patient care services and support to hospice patients and their families, ensuring a trusting and compassionate relationship.
Responsibilities
- Act as the primary liaison between physician, patient/family, and hospice team.
- Empower patients' caregivers through education and support.
- Manage interdisciplinary teams to provide comfort and dignity to patients.
- Evaluate patient and family needs and execute personalized care plans.
- Maintain patient clinical records and participate in quality assessment programs.
Requirements
- RN license in the state of requested employment.
- 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology preferred.
- Compassionate and empathetic approach to patient care.
- Current driver's license, automobile insurance, and CPR certification.
