RN Case Manager - Full-Time - Hospice ($5000 Sign On)

Heartland Hospice, a member of the Gentiva family, is seeking a compassionate Hospice Registered Nurse (RN) / RN Case Manager to join our team. In this role, you will provide direct patient care services, act as a liaison between physicians and patients/families, and ensure each patient receives personalized care in the comfort of their home.

Responsibilities

• Act as primary liaison between physician, patient/family, and hospice team.
• Provide direct patient care and supportive services to patients and families.
• Collaborate with an interdisciplinary team to ensure patients' comfort and dignity.
• Evaluate patient needs and contribute to individualized care plans.
• Maintain accurate patient clinical records and participate in quality assessment programs.

Requirements

• RN license in the state of employment (or eligibility to obtain).
• 1+ years of nursing experience in hospice, home health, ICU, or related fields.
• Compassionate, empathetic, and flexible approach to patient care.
• Current driver's license, automobile insurance, and CPR certification.
