Job Description
We are currently looking for a compassionate and dedicated RN Case Manager to join our team at Heartland Hospice, a part of the Gentiva family. As an RN Case Manager, you will have the opportunity to provide direct patient care services and support to hospice patients and their families, ensuring a compassionate and trusting relationship during a crucial time. In this role, you will work closely with physicians, patients, and the hospice team to deliver personalized care and maintain control and professional management of each patient's needs.
Responsibilities
- Act as the primary liaison between physicians, patients/families, and the hospice team.
- Provide direct patient care services and supportive care to patients and their families.
- Empower patient caregivers through education on caregiving techniques.
- Collaborate with an interdisciplinary team to ensure comfort, peace, and dignity for patients.
- Evaluate patient and family needs and assist in developing individualized care plans.
- Maintain accurate patient clinical records and participate in quality assessment programs.
Requirements
- RN license in the state of employment (or eligibility to obtain).
- 1+ years of nursing experience in hospice, home health, ICU, or related fields.
- Compassionate and empathetic attitude towards patients and families.
- Ability to provide care wherever the patient calls 'home'.
- Demonstrated flexibility and composure in crisis situations.
- Current driver's license, automobile insurance, and CPR certification.
