Job Description
We are seeking a compassionate and dedicated RN Case Manager to join our team at Heartland Hospice, a member of the Gentiva family. As an RN Case Manager, you will play a crucial role in providing physical, emotional, and spiritual support to patients and their families during challenging times.
Responsibilities
- Act as a primary liaison between physicians, patients/families, and the hospice team.
- Provide direct patient care services and supportive care to patients and families.
- Educate and empower patients' caregivers on caregiving best practices.
- Collaborate with an interdisciplinary team to ensure comfort, peace, and dignity for patients.
- Assist in developing and executing individualized care plans for patients.
- Maintain accurate patient clinical records and participate in quality improvement programs.
Requirements
- RN license in the state of employment (or eligibility to obtain).
- Compassionate, empathetic, and kind approach to patient care.
- 1+ years of nursing experience in hospice, home health, ICU, or related settings preferred.
- Current driver's license, automobile insurance, and CPR certification.
- Ability to provide care wherever the patient resides.
