Job Description
Heartland Hospice, a member of the Gentiva family, is seeking a compassionate and dedicated RN Case Manager to join our team on a full-time basis. As a Hospice RN Case Manager, you will play a crucial role in providing personalized care and support to patients and their families during challenging times, ensuring comfort, dignity, and quality of life.
Responsibilities
- Act as the primary liaison between the physician, patient/family, and hospice team.
- Provide direct patient care services and supportive care to patients and their families.
- Educate and empower patient caregivers on care techniques.
- Collaborate with an interdisciplinary group to ensure a comprehensive care plan.
- Maintain accurate patient clinical records and participate in quality assessment programs.
Requirements
- Compassionate and empathetic attitude towards patients and families.
- RN license in the state of employment.
- Minimum 1 year of nursing experience in hospice, home health, ICU, or related fields.
- Strong communication and interpersonal skills.
- Current driver's license and CPR certification.
