Job Description
Heartland Hospice, a member of the Gentiva family, is looking for a dedicated Full-Time RN Case Manager to join our team. As an integral part of our mission to provide compassionate care to patients and their families, you will have the opportunity to make a significant impact when it matters most.
Responsibilities
- Act as the primary liaison between physicians, patients/families, and the hospice team.
- Provide direct patient care services with a focus on comfort and dignity.
- Educate and support patient caregivers throughout the care process.
- Collaborate with interdisciplinary teams to ensure comprehensive care.
- Maintain accurate patient clinical records and participate in quality assessment programs.
Requirements
- RN license in the state of employment.
- 1+ years of relevant nursing experience preferred.
- Compassionate and empathetic with a hospice heart.
- Ability to provide care in various settings.
