Job Description
Heartland Hospice, a member of the Gentiva family, is seeking a compassionate and empathetic RN Case Manager to join our team on a full-time basis. In this role, you will provide direct patient care services to hospice patients and their families, offering comfort, peace, and dignity during challenging times. As a key member of our team, you will have the opportunity to make a meaningful difference in the lives of those in need.
Responsibilities
- Act as the primary liaison between physicians, patients/families, and the hospice team.
- Empower patient caregivers through education and support.
- Evaluate patient and family needs to develop and execute personalized care plans.
- Maintain accurate patient clinical records and participate in quality assessment programs.
Requirements
- RN license in the state of employment or eligibility to obtain.
- 1+ years of nursing experience in hospice, home health, ICU, or related fields preferred.
- Compassionate and empathetic approach to patient care.
- Current driver's license, automobile insurance, and CPR certification.
