Job Description
Heartland Hospice, a member of the Gentiva family, is seeking a compassionate and dedicated RN Case Manager to join our team on a full-time basis. In this role, you will provide direct patient care services to hospice patients and their families, ensuring comfort, peace, and dignity throughout their journey. Responsibilities
- Act as the primary liaison between physician, patient/family, and hospice team.
- Provide direct patient care services and supportive care to patients and families.
- Educate and empower patient caregivers on how to care for their loved ones.
- Collaborate with interdisciplinary teams to ensure comprehensive care.
- Participate in quality assessment and performance improvement programs.
Requirements
- Compassionate and empathetic demeanor towards patients and families.
- RN license in the state of employment (or eligibility to obtain).
- 1+ years of nursing experience in hospice, home health, ICU, or related fields preferred.
- Current driver's license, automobile insurance, and CPR certification.
