Job Description
We are looking for a compassionate and dedicated RN Case Manager to join our team at Heartland Hospice. As an integral part of our hospice care services, you will have the rewarding opportunity to provide direct patient care and support to individuals and their families during their most challenging times.
Responsibilities
- Act as the primary liaison between physicians, patients, and the hospice team.
- Provide direct patient care services and supportive care to patients and families.
- Educate and empower patients' caregivers in providing care.
- Collaborate with an interdisciplinary team to ensure comfort and dignity for patients.
- Evaluate patient needs and assist in developing care plans.
- Maintain accurate patient clinical records and participate in quality assessment programs.
Requirements
- Compassionate and empathetic approach to patient care.
- Active RN license in the state of employment.
- At least 1 year of nursing experience in hospice, home health, or related fields.
- Flexibility, composure, and crisis management skills.
- Current driver's license, automobile insurance, and CPR certification.
