Job Description
Join our team at Heartland Hospice, a proud member of the Gentiva family, as a Full-Time Hospice RN Case Manager. We offer a $5,000 Sign-On Bonus and a comprehensive benefits package. As an RN Case Manager, you will be the primary liaison between physicians, patients, and families, providing compassionate care and ensuring the highest quality of life for our patients. Collaborate with an interdisciplinary team and contribute to our quality assessment and improvement programs.
Responsibilities
- Provide direct patient care and support to hospice patients and their families.
- Act as the primary coordinator between all care stakeholders.
- Educate patients and caregivers on care protocols and support systems.
- Maintain accurate patient records and participate in quality assessment programs.
Requirements
- RN license in the state of employment.
- Compassionate, kind, and empathetic approach to patient care.
- 1+ years of nursing experience, preferably in hospice or related fields.
- Flexible and composed in crisis situations.
- Current driver's license, automobile insurance, and CPR certification.
