Job Description
We are looking for a compassionate and dedicated RN Case Manager to join our team at Heartland Hospice, part of the Gentiva family. As a Full-Time RN Case Manager, you will play a crucial role in providing personalized care and support to hospice patients and their families.
Responsibilities
- Act as the primary liaison between physicians, patients, and the hospice team.
- Provide direct patient care services and support to patients and their families.
- Educate and empower caregivers to ensure the well-being of the patients.
- Collaborate with an interdisciplinary team to deliver compassionate care and maintain patient dignity.
- Evaluate patient needs and assist in developing care plans.
- Maintain accurate patient clinical records and participate in quality assessment programs.
Requirements
- RN license in the state of employment (or eligibility to obtain).
- 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology preferred.
- Compassionate, kind, and empathetic approach to patient care.
- Current driver's license and automobile insurance.
- Current CPR certification.
