Responsibilities

• Provide direct patient care services and support to hospice patients and their families.
• Act as the primary liaison between physicians, patients, and the hospice team.
• Educate and empower patient caregivers to ensure optimal care.
• Collaborate with interdisciplinary teams to deliver personalized care and maintain patient records.

Requirements

• RN license in the state of employment.
• Comfortable with home-based care.
• Compassionate, empathetic, and flexible approach.
• Prior experience in hospice, home health, or related fields preferred.
• Current CPR certification and valid driver's license.
