Heartland Hospice, a member of the Gentiva family, is seeking a dedicated RN Case Manager to join our compassionate team. As a Full-Time Hospice RN, you will provide personalized care and support to patients and their families during challenging times. Responsibilities

• Act as a primary liaison between physicians, patients/families, and hospice team.
• Deliver direct patient care services and supportive care to patients and families.
• Collaborate with interdisciplinary teams to ensure comfort and dignity for patients.
• Educate and support patient caregivers in providing care.
• Participate in quality assessment and improvement programs.

Requirements

• RN license in the state of employment.
• 1+ years of nursing experience, preferably in hospice or related fields.
• Compassionate, flexible, and empathetic approach to patient care.
• Current driver's license, automobile insurance, and CPR certification.
