Job Description
We are looking for a compassionate and experienced RN Case Manager to join our team at Heartland Hospice. As an integral part of our hospice care services, you will provide direct patient care, support families, and ensure comfort and dignity for those in need. Responsibilities
- Act as a primary liaison between physician, patient/family, and hospice team.
- Provide direct patient care services and supportive care to patients and families.
- Educate and empower patient caregivers on care techniques.
- Maintain patient clinical records and participate in quality assessment programs.
Requirements
- RN license in the state of employment.
- 1+ years of nursing experience in hospice, home health, or related fields.
- Compassionate, empathetic, and flexible approach to patient care.
- Current CPR certification, driver's license, and automobile insurance.
