Job Description
We are looking for a dedicated RN Case Manager to join Heartland Hospice, a leading provider of hospice, palliative, and home health care. As a Full-Time RN Case Manager, you will play a crucial role in providing compassionate care to patients and their families during challenging times. Responsibilities
- Act as the primary liaison between physician, patient/family, and hospice team.
- Provide direct patient care services and support to patients and their families.
- Educate and empower patients' caregivers on providing care for their loved ones.
- Collaborate with an interdisciplinary group to ensure comfort and dignity for patients.
- Evaluate patient needs and assist in developing personalized care plans.
- Maintain accurate and detailed patient clinical records.
- Participate in quality assessment and performance improvement programs.
Requirements
- Compassionate RN with hospice heart and empathy for patients and families.
- Ability to provide care at the patient's home with flexibility and composure in crisis situations.
- Active RN license in the state of employment (or eligibility to obtain).
- Minimum of 1 year of nursing experience in hospice, home health, ICU, geriatric, or oncology preferred.
- Additional experience in areas such as ER, critical care, wound care, or travel nursing is advantageous.
- Current driver's license and valid automobile insurance.
- Current CPR certification.
