Job Description
Heartland Hospice, a member of the Gentiva family, is seeking a full-time RN Case Manager to join our compassionate team. Responsibilities
- Directly report to senior management while maintaining patient care control.
- Act as a liaison between physicians, patients, and the hospice team.
- Provide direct patient care and support to families with empathy and trust.
- Educate and empower caregivers in providing optimal care.
- Collaborate with interdisciplinary teams to ensure comfort and dignity for patients.
- Evaluate patient needs and contribute to care planning and execution.
- Maintain accurate patient records and participate in quality assessment programs.
Requirements
- Compassionate and empathetic with patients and families.
- Hold a valid RN license.
- Minimum of 1 year nursing experience in hospice, home health, or related fields preferred.
- Flexibility and composure in crisis situations.
- Possess a current driver's license, automobile insurance, and CPR certification.
