Job Description
We are looking for a compassionate and dedicated RN Case Manager to join our team at Heartland Hospice, a member of the Gentiva family. In this full-time role, you will provide essential medical and emotional support to hospice patients and their families, helping them make the most of every moment.
Responsibilities
- Act as the primary liaison between physician, patient/family, and hospice team.
- Provide direct patient care services and supportive care to patients and families.
- Educate and empower patients' caregivers on providing care.
- Collaborate with an interdisciplinary team to ensure comprehensive care.
- Maintain patient clinical records and participate in quality assessment programs.
Requirements
- RN license in the state of employment (or eligibility to obtain).
- 1+ years of nursing experience, preferably in hospice, home health, or related fields.
- Compassionate, kind, and empathetic approach to patient care.
- Current driver's license and CPR certification.
