RN Case Manager - Full-Time - Hospice
$5000 Sign-On Bonus!

Responsibilities
- Provide direct patient care services to hospice patients and support their families.
- Act as a liaison between physicians, patients, and the hospice team.
- Educate and empower patients' caregivers on providing care.
- Manage interdisciplinary teams to ensure comfort and dignity for patients.
- Evaluate patient needs and contribute to care plans.
Requirements
- Compassionate and empathetic Hospice Registered Nurse with an active RN license.
- Previous experience in hospice, home health, or related fields preferred.
- Flexibility, composure, and crisis support skills.
- Current driver's license, automobile insurance, and CPR certification.
