Job Description
We are looking for a compassionate and experienced RN Case Manager to join our team at Heartland Hospice, a member of the Gentiva family. As a Full-Time Hospice RN, you will play a crucial role in providing holistic care and support to patients and their families during challenging times.
Responsibilities
- Act as the primary liaison between physicians, patients/families, and the hospice team.
- Provide direct patient care services and support to patients and their families with empathy and compassion.
- Educate and empower caregivers on providing care to their loved ones.
- Collaborate with a multidisciplinary team to ensure comfort and dignity for patients.
- Participate in quality assessment and performance improvement initiatives.
Requirements
- RN license in the state of employment (or eligibility to obtain).
- Compassionate and empathetic approach to patient care.
- 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology preferred.
- Current driver's license, automobile insurance, and CPR certification.
