RN Case Manager - Full-Time - Hospice ($5000 Sign On)

Job Description

We are looking for a compassionate and experienced RN Case Manager to join our dynamic team at Heartland Hospice, a member of the Gentiva family. As a Full-Time Hospice RN, you will have the opportunity to make a significant impact on the lives of patients and their families during challenging times.

Responsibilities

• Act as the primary liaison between physician, patient/family, and hospice team.
• Provide direct patient care services and support to patients and their families.
• Educate and empower patients' caregivers on caregiving techniques.
• Collaborate with an interdisciplinary team to ensure comfort, peace, and dignity for patients.
• Evaluate patient needs and contribute to the development and execution of care plans.
• Maintain accurate patient clinical records and participate in quality assessment programs.

Requirements

• Compassionate and empathetic with a hospice heart.
• RN license in the state of employment or eligibility to obtain.
• 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology preferred.
• Current driver's license, automobile insurance, and CPR certification.
• Demonstrated flexibility, composure, and crisis management skills.
