Schedule: Monday – Friday 7:30 AM – 4:00 PM plus limited on-call rotation
Location: Homestead, PA or Wilkinsburg, PA
Summary of Job Duties: If you are an RN with a background in home care or hospice and have a passion for managing geriatric care-planning in a team environment, the RN Case Manager (RNCM) role might be the perfect fit for you. The RNCM provides in-home assessments and overall care-management to frail elders aiming to live at home. This role involves assessing and coordinating nursing and home care services, local travel for in-home assessments, patient visits, interdisciplinary meetings, and office time for documentation.
Required Education & Experience: Graduate of an accredited nursing school with at least one year of experience working with a frail elderly population, preferably in long-term care, home health care, or community health setting. Experience in teaching patients and healthcare workers is preferred.
Required Certifications and Licensure: Licensed Registered Nurse in Pennsylvania. Valid Driver’s License.
Required Skills:
- Comprehensive knowledge of current nursing practice and theory
- Knowledge of the medical, social, and emotional needs of a frail, elderly population
- Effective written and oral communication skills
- Strong organizational and planning skills; ability to manage multiple priorities
- Ability to work independently and make critical decisions
- Working knowledge of utilization review, quality assurance, and managed health care concepts
- Ability to delegate responsibility and work with the interdisciplinary team approach to elderly care
- Knowledge of local health care and geriatric service networks
- Basic working knowledge of Windows operating systems, e-mail, and word processing
