Rehabilitation Therapist Responsibilities:

• Conduct home visit assessments of referred patients to assess medical conditions and functional capabilities.
• Provide direct patient care and develop rehabilitation plans.
• Proficiently document patient information in an Electronic Medical Record (EMR) system.

Requirements:

• Graduate from an accredited Physical Therapy (PT) program.
• Hold an unencumbered PT license for New York.
• Possess CPR Certification.
• Experience with EMR documentation preferred.
