Rehabilitation Therapist Responsibilities:

• Assess patients' medical conditions and functional capabilities through home visits.

• Proficient in EMR documentation.
• Maintain CPR Certification.

Rehabilitation Therapist Requirements:

• Graduate from an accredited Physical Therapy (PT) program.
• Hold an active Physical Therapy (PT) license for New York.
• Experience with EMR systems, preferably Hello Note.
• Strong interpersonal skills and compassion for patient care.
