• Perform home visit Physical Therapy assessments for referred patients.
• Administer direct patient care to evaluate medical conditions, functional capabilities, limitations, and rehabilitation potential.
• Proficiency in EMR documentation.
• Maintain CPR Certification.

Requirements:

• Graduated from an accredited Physical Therapy program.
• Unencumbered Physical Therapy license for New York.
