• Perform home visits to conduct Physical Therapy assessments, provide direct patient care, and evaluate medical conditions.
• Document patient information using EMR systems like Hello Note (experience not mandatory).

Requirements:

• Graduate from an accredited Physical Therapy program.
• Hold an unrestricted Physical Therapy license for New York.
• Possess CPR certification.
