Paradigm Rehab is seeking a dedicated Rehabilitation Therapist to help individuals enhance their quality of life through reconnecting with their environment. Responsibilities

• Conduct home visit assessments to evaluate patients' medical conditions and rehabilitation potential.
• Provide direct patient care and develop treatment plans tailored to each individual's needs.
• Document patient progress accurately in our EMR system.
• Collaborate with healthcare professionals to promote optimal patient outcomes.

Requirements

• Graduate from an accredited Physical Therapy program.
• Hold a valid Physical Therapy license in New York.
• Proficiency in EMR documentation preferred.
• CPR Certification.
