Job Description
We are looking for a dedicated and experienced Registered Nurse (RN) Field Nurse to join our Home Care team. As part of our organization, you will provide skilled nursing care and treatment to geriatric patients in the community. Our state-of-the-art Electronic Health Record system and comprehensive training ensure that you have the tools to excel in your role.
Responsibilities
- Provide skilled nursing care and treatment to geriatric patients in the community.
- Develop and implement goal-oriented care plans consistent with available resources.
- Facilitate the discharge planning process for patients.
Requirements
- BSN required; ADN candidates must obtain BSN within three years of start date.
- Current DE RN license or compact state license.
- 2 years of Home Health RN experience within the last 12 months.
- IV proficiency required or willingness to train.
