Job Description

We are looking for a compassionate and skilled Registered Nurse (RN) Field Nurse to join our dynamic team focusing on Home Care. If you have experience in Home Health nursing and are passionate about providing care to geriatric patients, this opportunity is for you. Responsibilities

• Provide skilled nursing care and treatment to geriatric patients in the community.
• Develop and implement patient-centered care plans.
• Ensure IV proficiency, with training provided if needed.
• Create discharge plans for patients.

Requirements

• BSN required; ADN candidates must obtain BSN within 3 years.
• Current DE RN license or compact state license.
• Minimum of 2 years of recent Home Health RN experience.
• Prior experience in Home Healthcare and Med-Surgical settings preferred.
