Job Description
Heartland Hospice, a member of the Gentiva family, is seeking a compassionate and dedicated Registered Nurse On Call to join our team. You will provide direct patient care services and support to patients and families outside normal business hours, ensuring they receive the care they need.
Responsibilities
- Deliver direct patient care services and supportive care on an on-call basis.
- Manage an interdisciplinary team to enhance the quality of hospice care.
- Evaluate patient and family needs, and assist in developing and implementing care plans.
- Maintain accurate patient clinical records and participate in quality assessment programs.
Requirements
- Current RN license (or ability to obtain one) in the state of employment.
- Minimum of 1 year nursing experience in hospice, home health, or oncology preferred.
- Compassionate, empathetic, and dedicated to providing quality care.
