Job Description

Responsibilities
- Provide direct patient care services to hospice patients and offer supportive care to their families.
- Manage an interdisciplinary group to enhance the quality of hospice care.
- Evaluate patient/family needs and contribute to the development and implementation of individualized care plans.
- Maintain accurate patient clinical records and participate in quality assessment programs.
Requirements
- Current RN license.
- Minimum of 1 year nursing experience in hospice, home health, or oncology.
- Compassionate and empathetic towards patients and families.
