Job Description

As a Registered Nurse, you will play a crucial role in delivering quality patient care and support to individuals and their families during challenging times. Reporting to the Administrator, Executive Director, or Patient Care Manager Senior, you will manage patient care, act as a primary liaison, and lead an interdisciplinary team to ensure the best possible hospice services.
Responsibilities
- Provide direct patient care and support to hospice patients and their families.
- Lead an interdisciplinary team to enhance the quality of care.
- Evaluate patient needs and contribute to individualized care plans.
- Maintain accurate patient clinical records and participate in quality assessment programs.
Requirements
- Current RN license or ability to obtain one.
- Minimum of 1 year nursing experience in hospice, home health, or oncology.
- Compassionate, empathetic, and dedicated to patient well-being.
