Job Description
Gentiva Hospice, a leading provider of hospice, palliative, home health, and personal home care services, is seeking a compassionate Registered Nurse to join our dedicated team. As a Registered Nurse, you will play a crucial role in providing direct patient care services, managing interdisciplinary teams, and ensuring top-quality hospice care for our patients.
Responsibilities
- Provide direct patient care services to hospice patients and offer supportive care to their families.
- Manage an interdisciplinary group including Licensed Practical/Vocational Nurses, Certified Nursing Assistants, and support staff to enhance the quality of care.
- Evaluate patient and family needs, and contribute to the development and execution of individualized care plans.
- Maintain accurate clinical records for each assigned patient.
- Participate in quality assessment and performance improvement programs.
Requirements
- Current RN license in the state of employment or eligibility to obtain it.
- Minimum of 1 year of nursing experience in hospice, home health, or oncology settings.
- Compassionate, empathetic, and dedicated to providing excellent care to patients and their families.
