Job Description
We are looking for a compassionate and empathetic Registered Nurse to join our team at Gentiva Hospice. In this role, you will provide direct patient care services, manage an interdisciplinary group to enhance hospice care quality, and work as a primary liaison between physicians, patients, and families.
Responsibilities
- Provide direct patient care and supportive services to hospice patients and their families.
- Manage an interdisciplinary team to optimize the quality of care provided.
- Evaluate patient and family needs and assist in the development and implementation of individualized care plans.
- Maintain accurate and detailed patient clinical records.
- Participate in quality assessment and performance improvement initiatives.
Requirements
- Compassionate, kind, and empathetic towards patients and families.
- Active RN license in the state of employment or eligibility to obtain one.
- Minimum 1 year of nursing experience in hospice, home health, or oncology.
