Job Description
We are looking for a compassionate and experienced Registered Nurse to join our team at Community LIFE. As an RN Case Manager, you will be responsible for providing in-home assessments and care management for frail elders, supporting their goal of independent living. This role involves coordinating nursing and home care services, conducting in-home assessments, and participating in interdisciplinary meetings and care planning.

Schedule
Monday - Friday, 7:30 AM - 4:00 PM, with limited on-call rotation.

Required Qualifications
- Graduate of an accredited nursing school.
- Minimum 1 year of experience with a frail elderly population.
- Pennsylvania Registered Nurse license.
- Valid Driver's License.
