Job Description
We are looking for a passionate Registered Nurse to join our team at Gentiva Hospice, a leading provider of hospice, palliative, home health, and personal home care services. As a Registered Nurse, you will play a crucial role in providing physical, spiritual, and emotional support to patients and their families, ensuring they receive the best care possible.
Responsibilities
- Provide direct patient care services to hospice patients with a reasonable case load
- Act as the primary liaison between physicians, patients/families, and the hospice team
- Manage an interdisciplinary group to enhance the quality of hospice care
- Evaluate patient/family needs and assist in the execution of care plans
- Maintain accurate patient clinical records and participate in quality assessment programs
Requirements
- Current RN license or ability to obtain one in the state of requested employment
- Minimum of 1 year of nursing experience in hospice, home health, or oncology
- Compassionate, empathetic, and dedicated to providing exceptional care
