Job Description
We are seeking a compassionate and dedicated Registered Nurse to join our team at Gentiva Hospice, a renowned industry leader providing hospice, palliative, home health, and personal home care. As a Registered Nurse, you will play a crucial role in delivering high-quality patient care and support to individuals and their families during challenging times.
Responsibilities
- Provide direct patient care services and support to hospice patients and their families.
- Manage an interdisciplinary team to enhance the quality of care.
- Evaluate patient/family needs and contribute to the development and execution of individualized care plans.
- Maintain accurate and up-to-date patient clinical records.
- Participate in quality assessment and performance improvement programs.
Requirements
- Current RN license or ability to obtain one.
- Minimum of 1 year nursing experience in hospice, home health, or oncology.
- Compassionate, empathetic, and dedicated to providing exceptional patient care.
