Job Description

Responsibilities
- Provide direct patient care services to hospice patients and their families.
- Manage an interdisciplinary team to ensure high-quality care.
- Evaluate patient needs and contribute to the care plan.
- Maintain accurate patient clinical records and participate in quality assessment programs.
Requirements
- Current RN license or ability to obtain one.
- Minimum 1 year of nursing experience in hospice, home health, or oncology.
