Responsibilities

• Provide and document skilled nursing care in accordance with care plans and physician orders.
• Observe and assess patient conditions, reporting changes to supervisors and physicians.
• Participate in care planning and take appropriate nursing actions based on assessments.
• Educate patients and families on disease processes, self-care, and prevention strategies.
• Maintain knowledge through educational programs and skill evaluations.

Requirements

• 6 months of nursing experience.
• Active nursing license and CPR certification.
• Strong communication skills, ability to work collaboratively with a team.

Physical Requirements

• Must be able to lift 50 pounds and perform various physical activities.
• Visual, auditory, and sensory acuity required.
• Ability to respond to emergency situations in the home or during transport.
