Responsibilities

• Deliver and coordinate quality patient care as per physician orders.
• Observe, assess, and report patient condition changes.
• Document all care activities and maintain care plans.
• Implement infection control measures and patient education.

Requirements

• Graduate of an accredited nursing school with current state LPN license.
• CPR certification and proficiency in clinical assessments.

Preferences

• 6 months of recent clinical experience as an LPN.
• Previous home health experience.
