Physician in Geriatric Care - Elder Services Day Program (PACE Program) - SE Dartmouth, MA

Job Description

We are looking for a dedicated Physician in Geriatric Care to join our Elder Services Day Program at the Summit ElderCare PACE Program in SE Dartmouth, MA. As part of our team, you will have the opportunity to provide exceptional care to older adults in a comprehensive, interdisciplinary setting.

Responsibilities

• Devote time to meet the clinical and social needs of program participants.
• Work collaboratively with a team of geriatric care professionals.
• Participate in the Interdisciplinary Team (IDT) meetings for care planning.

Qualifications

• Medical degree with a specialization in Geriatrics preferred.
• Board certification in Internal Medicine, Geriatrics, or Family Practice desired.
• Minimum of two years of experience working with frail or elderly populations.
