• Conduct home visits to assess patients' medical conditions and functional capabilities.
• Provide direct patient care and evaluate potential for rehabilitation.
• Proficient in EMR documentation.

Requirements

• Graduate from an accredited Physical Therapy (PT) program.
• Hold an unencumbered Physical Therapy (PT) license for New York.
• CPR Certification.
