Responsibilities

• Conduct home visit assessments and provide direct patient care to evaluate medical conditions and rehabilitation potential.
• Utilize EMR documentation for accurate record-keeping.
• Work collaboratively with patients to improve functional capabilities.

Requirements

• Graduated from an accredited Physical Therapy program.
• Hold an unencumbered Physical Therapy license for New York.
• Proficient in EMR systems; experience with Hello Note is a plus.
• CPR Certification.
