Physical Therapy Specialist Requirements:

• Graduated from an accredited Physical Therapy (PT) college or university.
• Unencumbered Physical Therapy (PT) license for New York.
• Conduct home visit assessments of referred patients providing direct patient care.
• Proficient in EMR documentation (experience in Hello Note is a plus).
• CPR Certification.
