• Conduct home visit assessments for patients, evaluating medical conditions and potential for rehabilitation.

• Document patient information accurately using our EMR system.

Requirements

• Graduate of an accredited Physical Therapy program.
• Licensed Physical Therapist in New York.
• CPR Certification.
• Experience in home health is preferred.
