Physical Therapy Specialist Requirements:

• Graduate from an accredited Physical Therapy program.
• Hold an unencumbered Physical Therapy license for New York.
• Perform home visit assessments, provide direct patient care, and evaluate medical conditions and rehabilitation potential.
• Proficiency in EMR documentation (experience with Hello Note is a plus).
• Hold a CPR certification.
