Physical Therapist Requirements:

• Graduated from an accredited Physical Therapy (PT) college or university.
• Hold an unencumbered Physical Therapy (PT) license for New York.
• Conduct home visits to assess patients' medical conditions, functional capabilities, and potential for rehabilitation.
• Proficiency in EMR documentation, preferably Hello Note.
• CPR Certification.
