Physical Therapist Requirements

• Graduated from an accredited Physical Therapy (PT) college or university.
• Hold an unencumbered Physical Therapy (PT) license for New York.
• Conduct home visit Physical Therapy assessments to evaluate medical conditions and rehabilitation potential.
• Proficiency in EMR documentation; experience with Hello Note is a plus.
• Current CPR Certification.
